Convenience: Respirator medical evaluation for ALL brands and ALL types of respirators (per OSHA Standard 29CFR1910.134) Your employees do NOT need to leave the work site. Our Respirator Medical Evaluation Questionnaire is available in English or Spanish
Speed: Log-in or dial-in! Whether they utilize the online service or our unique IVR service, it takes most employees only 7 to 10 minutes to complete the questionnaire
Consistency: All employees are evaluated using the same expanded medical survey
Quality: Board-certified medical physicians complete the review
Management: If indicated, a further examination and Pulmonary Function Test is arranged through a facility local to your employee (Required for only about 3% or fewer (3 or fewer out of every 100) of all evaluations. Arranged by us for your employee locally for just $39.00 anywhere in the U.S., no mater our cost)
It’s As Easy As 1-2-3 for you AND for your employee!
1. Using any computer with Internet access, the employer (as “Administrator”) creates company “respirator” profile(s) and is assigned a company code. Our system allows credit card charge per certification OR monthly summary billing. Billing of ONLY $25 PER CERTIFICATION ISSUED (no charge if no certification issued)
2. The employer then creates one or more Respirator Profiles describing the respirator type, the work being done and conditions under which the respirator(s) is/are used.
3. Each employee logs on to the secure web site – or dials-in to an 800# – using your company code, then confidentially fills out an online questionnaire or answers all questions by IVR and submits it for evaluation. Per OSHA regulations, employees have access to OHS, Inc. physicians to ask any questions.
Employer AND Employee receive email notification of his/her respirator use status within 2-4 business hours after completion of the evaluation questionnaire.
The Administrator’s web page allows employers to review all employees alphabetically, search by resullts and certifications by location, the type of respirator worn, or search for a particular employee. Additional available information includes: date questionnaire completed, employee status (cleared, pending review, denied, etc.)
